Login Issue OJS 3.2.1.4

Journal website is behaving abnormally. upon Login it takes back to the login screen or if it logs in, it gives error in a Modal. “You are not authorized to access the requested resource” and then
“An unexpected error has occurred. Please reload the page and try again.”

Steps I took leading up to the issue:
I have already set session_check_ip to off and changed session cookie name in config.inc.php but to no avail.

What I tried to resolve the issue:
