Editorial history - mixed up upload information

In the editorial history, we are getting mixed up upload history. Example submission id #5522 shows an image upload of #5523. The file was uploaded by different authors.
See the editorial history of #5522. Te 3rd file is of #5523 by Mat Suraj, not S. Gokula.

Hi @latha_ojs,

Is there any relevant maintenance history for your installation that might narrow down the circumstances? For example, was there an upgrade involved from a previous version?

I don’t think so. Actually it was working fine after upgrading that was in September.

Actually, I noticed that information keeps changing particularly it happens when a submission fail.

Hi @latha_ojs,

Is it just the submission log view that’s incorrect, or does the submission actually contain e.g. files from other submissions somehow?
