Cover Image for OMP does not appear on front end

Cover images for books in OMP does not appear on front end after publishing. How do I fix this? I get an “unexpected error” message when I upload the image under “Metadata” and click on “Save”, but the message disappears when I try a second time. Still, no image appears on front end.

OMP version is 3.4.0.4.

Do you see any errors in your PHP error log? If so, please share them here. That will provide more clues as to what the issue might be.
