3.1.1.4 upload confirmation fails - urgent help

Hi,
Suddenly we are not getting upload file confirmation. The files uploaded and I can see in the files folder but don’t get confirmation. I don’t see any error in the error_log. Please help.

Before this, I was trying to upgrade to 3.1.2.1 and I got some problem with event_log and email_log. I think some problem with my event_log table structure. But I see the records for every event.

my event_log and event_log_settings table structure giving the problem. When I remove indexes from event_log and event_log_settings upload confirmation works fine. Please let me know what is the exact table structure for event_log and event_log_settings.
