This looks like the same issue others have encountered in these threads:
- OJS Upgrade Error: from 3.2.1.4 to 3.3.0-15
- Upgrade failed: DB: SQLSTATE[23000]: Integrity constraint violation: 1062 Duplicate entry ‘index-6-blockTitle’ for key ‘plugin_settings_pkey’
I’d suggest trying the solutions recommended there.
