Using OCS - Error when reviewer clicks on the icon next to Review

I get the following error in the popup window when a reviewer clicks on the icon next to ‘Review’:

This page isn’t working
server is currently unable to handle this request
HTTP ERROR 500

I don’t know how I can rectify this. Help!
