Notify Authors and Reviewers E-mail Sending Related Problem

When I go for Request Revision and Notify to Author and Reviewer by clicking Record Decision. I am getting the error: An unexpected error has occurred. Please reload the page and try again. What is the reason behind it and solution. Please give me a suggestions what should I do? Advanced Thanks !!!

We are using: OJS 3.4.0-4
