Hi @radjr,

Depending on what changes you want to make, you’ll likely need to modify the template – off the top of my head, that’s templates/subscription/userIndividualSubscriptionForm.tpl and the associated class classes/subscription/form/UserIndividualSubscriptionForm.inc.php.
