Can you describe specifically how you added the file upload? I’m still interested in resolving the original issue, where the required fields message was causing trouble.
We’re trying to build a knowledge base using this forum, and I try to avoid providing free support via private messages if possible, as it’s not helpful to the broader community. Could you post your additional question as a new topic?

What I would ask is that you answer me as I can erase the duplication in the review forms (attached capture what I want to solve)

You can’t delete these review forms because they’ve been used for reviews. Deleting the review form would mean deleting the review response, and that would break the scholarly record for those submissions. Only unused review forms can be deleted.
