Anonimity during the review process when using customed review form

Steps I took leading up to the issue

  1. I have created a customed review form.
  2. When reviewer uses it and wants to add a comment for author (or when the author wants to answer to the reviewer’s comment, the anonimity is being uncovered, as the name(s) of reviewer or/and author begin to be visible.
  3. Is there any way to fix this issue? We use double blind review approach. Thank you in advance for your time and assistance.
